If you are paying by check or need a printable registration form:
Print this registration form, complete it and mail or fax it with your payment. The fax number and mailing address are located on the form. Payment by mail must be received NO LATER than 13 February 2008. If payment is not received by 13 February 2008, you will need to register on-site.
You will not be registered for the conference until payment is received.
Cancellations and Substitutions
If you must cancel for any reason, notify our registration department by 18 January 2008. Your registration will be refunded less a £100 processing fee. Cancellations after January 18, 2008 are non-refundable. You may transfer your registration to another person at any time by providing written authorization.
